I certify that I am the authorized user of the credit card for the above reservation. I grant Hominy Ridge Cabins and Gift Shop permission to charge my credit card accordingly so long as it directly relates to the services rendered or for damages that occur during me and my guests stay. I will do my best to ensure that the cabin and hot tub are taken care of according to the rules and policies that Hominy Ridge Cabins and Gift Shop have given to me in all correspondence and hereby sign below. I agree to have Hominy Ridge Cabins and Gift Shop place a $250 damage deposit on my credit card at the time of check-in. I am aware that this damage deposit is for use at their descretion for damages that may occur during my stay. Hominy Ridge Cabins and Gift Shop agree to remove this authorization in a timely manner after cabins have been inspected and cleaned in the event that there are no reason for charges to occur.

PRIVATE SPA RULES AND WATER SAFETY We clean, sanitize and perfectly balance the mountain water in your private hot tub before every single arrival. To ensure a safe, crystal-clear experience for your stay, please follow these strict policies. • Registered Guests Only: Only the specific guests listed on this reservation are permitted in the hot tub. • Clean Bodies & Clean Suits: Please take a quick freshwater shower before stepping in to the hot tub to remove body lotions, sweat, and perfumes. Give swimsuits a quick rinse to remove laundry detergent. • Approved Attire Only: Only clean, standard swimwear is permitted. No street clothes, cotton t-shirts, or undergarments. • No River Wear: Bathing suits worn in the local river contain dirt and bacteria and are strictly prohibited from entering the spa. • No Glass Products: Glass breaks easily on porches and hot water and pose as a massive safety hazard. Please use plastic cups or cans only. • No Additives or Intimacy: Please keep all outside soaps, bubbles bath, bath bombs, and oils completely out of the water. For health and sanitation reasons, intimacy/sex is strictly prohibited in the spa. • Video Proof Water: We have taken a video of the water prior to your check-in. And will do so again after you check-out. Videoing is only done when the cabin is vacant! Before Check-in & After Check-out. Never during a guests stay! (Please Note: Normal, relaxed hot tub use is completely covered! A $200 remediation fee only applies to negligent water damage requiring a full 10-mile round trip mountain water haul and emergency drain/fill, heating, and chemical balancing.) (Children are to be guided along every step. This hot tub is not a bath tub and should not be used as such.) Thank you for helping us keep your cabin and hot tub perfect! Management

DOG POLICY ~NO DOGS in Cabin 1 and Cabin 3. No other animals are permitted at any time in any cabin. No Cats, birds, rabbits, etc. ~All dogs must be pre-approved before making reservations. ~There is a 2 dog limit and a 75 ( /-) pound weight limit. $15 per pet per night ~Dogs are Not permitted on the furniture or in beds. ~Dogs are Not permitted to be left alone in the cabin at any time, unless they are crated. We prefer that the dog(s) go with you on all your trips. ~Dogs must be crated at night. Our cabins are not to be used as kennels for dogs. ~Please have your dog groomed so there is less shedding and no fleas and ticks. It is your responsibility to clean up after your dog if it sheds. ~Any dog that has excessive shedding is not permitted. ~Please have your dog up-to-date with all shots. Have proof of vaccinations and a photo of your pet(s) handy to aid in finding any lost pet. ~Excessive barking and aggressive or threatening behavior will not be tolerated. ~No Aggressive breeds. ~All dogs MUST be registered with the Office and licenses, and inoculation records submitted if requested. ~All dogs must be on a leash at all times and no longer than six (6) feet when outside and under the control of the handler. ~No dog may be tied outside unattended. ~Dog owners MUST pick up after their pets in all areas. ~Dogs must not disturb other guests with excessive noise or aggressive or threatening behavior. ~Dogs are not allowed at/or in any public gathering locations and are not allowed within the seating areas of these facilities. Dogs are NOT allowed in the office/store or playground area. ~PLEASE BE COURTEOUS, USE YOUR OWN AREA FOR DOGGIE BUSINESS, AND CLEAN IT UP! ~The management reserves the right to prohibit any resident or guest from keeping a dog if said guest fails to observe one or more of these rules. -Visitors are not permitted to bring dogs onto Hominy Ridge Property unless pre-approved by owners. Cabin renter will be held responsible.
